Patient Details
A 50 year old male ,daily labour,resident of Nalgonda presented with
Chief Complaint 
Pain in the region of kidney 
Latest duration:since 2 months
Old duration:1year back
History of present illness
Patient was apparently alright 1 year back,then there was onset of pain in the kidney region.Since 1 month dialysis is being done.1 year back the pain was relieved by taking medicines,after stopping taking of medicines there was onset of pain again since 2 months
Past History
Patient took medicines for the kidney pain one year back
Non Diabetic
difficulty in breathing since 2 months hypertension since 2 months
Family History
No history of similar complaints in the family
Personal History
Diet-vegeterian
Sleep-adequate
Appetite -normal
Bowel and bladder -abnormal
General Examination
Patient was moderately built and moderately nourished
Pallor-present
No Icterus
No Cynosis
No clubbing
Presence of non pitting edema
Q/A
1) How many episodes of dialysis should I take in a week.
2)can this be cured without dialysis

This also reflects my patient centered online learning portfolio.                  CASE HISTORY Patient Details A 21 years old male , resident of choutuppal, presented with Chief Complaint *Severe vomitings since 2 weeks *Stomach pain since 2 weeks *Shortness of breath 1 week History of present illness *Patient was assymptomatic 2 weeks back,later there was onset of, more than 10 episodes of vomiting in 24 hours with nausea .
